H1N1 Flu Information for FPEH Undergraduate Students

Please refer to the full FPEH H1N1 Flu Preparedness Document for information about the procedures to follow should you become sick with flu-like symptoms.  A summary is provided below:

1) As soon as you feel sick stay at home, do not attend classes, exams or placements
2) For information on your symptoms contact Telehealth Ontario at 1-866-797-0000
3) Record your absence on ROSI using the new absence tool
4) Contact the Undergraduate Program Office and inform us of your absence

The Faculty of Physical Education & Health will provide up to date information to students during any disruption of normal education activities caused by a pandemic through the Faculty’s web site.

Missed Exams

FPEH Mid-terms
Students who miss an FPEH mid term test or examination for unforeseen and petitionable reasons must notify the Undergraduate Program Office at undergrad.peh@utoronto.ca within 24 hours of the scheduled test/examination. If the office is not notified within this time frame, students may receive a grade of zero for that test/examination.

A student may defer mid term examinations for only three reasons:
1. documented medical grounds;
2. documented CIS and OUA league and final competitions, documented international competition with a national team or other activities with equivalent significance, or;
3. documented compassionate grounds.

In each of these cases the FPEH Undergraduate Program Office must receive notification within the 24 hour time limit. Students submitting missed exam reports based on medical grounds are required to use the University of Toronto Student Medical Certificate, copies are available at the FPEH Undergraduate Office and Main Office or it can be downloaded from the web. Other medical notes will not be accepted. It is the responsibility of the student to contact the professor promptly to arrange for the make-up mid term examination.

Students who write a scheduled examination will not be allowed to ask for an alternative assessment or special consideration in marking after the fact.

Arts & Science Mid-terms
In the Faculty of Arts & Science the deferral of term tests falls under the jurisdiction of the course instructor. FPEH students are advised to report missed Arts & Science mid term exams with supporting documentation to all instructors as early as possible. More information is available at the Arts & Science web site

Missed Final Exams
All missed FPEH and Arts & Science final examinations must be reported to FPEH Undergraduate Program Office at undergrad.peh@utoronto.ca within 24 hours of the scheduled test/examination. If the office is not notified within this time frame, students may receive a grade of zero for that test/examination.

A student may defer a final examination for only three petitionable reasons:
1. documented medical grounds;
2. documented international competition with a national team or other activities with equivalent significance, or;
3. documented compassionate grounds.

In each of these cases the FPEH Undergraduate Program Office must receive notification within the 24 hour time limit. Students submitting missed exam reports based on medical grounds are required to use the University of Toronto Student Medical Certificate, copies are available at the FPEH Undergraduate Program Office and Main Office or it can be downloaded from the web. Other medical notes will not be accepted.

Deferred examinations from the December final exam period will be scheduled during the last two weeks in January. Deferred examinations from the April final exam period will be scheduled during the first two weeks in May. Students who miss writing deferred exams for petitionable reasons will write the make up exam at the next available opportunity which may be the following year when the course is offered again.

Students who write a scheduled examination will not be allowed to ask for an alternative assessment or special consideration in marking after the fact.

Final Exam Conflicts

If you have two or more final examinations scheduled for the same day and time and/or three final examinations scheduled within three consecutive examination time blocks, you have a final exam conflict. Students must report examination conflicts using the Exam Conflict Report form by the deadline date indicated on the posted FPEH Examination Schedule. Arrangements will be made for students to write conflicting examinations on the same day with a short supervised break between examinations or a deferred exam will be scheduled for one of the tests.

Exam Procedures and Guidelines

All students must have their T-Card identification displayed during any examination. The invigilators are to compare the signatures on the cards with those signed by the students on the official examination list. Students appearing without their cards must report to the FPEH Undergraduate Program Office within 24 hours to confirm their identity. Students failing to do so will be assessed a mark of zero on the examination.

What are the guidelines for an exam?

  • Students are advised to arrive at the examination room at least 15 minutes before the scheduled exam time
  • Students will not be permitted to enter the examination room later than 15 minutes after the start of the exam, nor to leave except under supervision until at least half an hour after the exam has commenced.
  • A pre-assigned seating plan will be used in all mid-term and final examinations.
  • Only a pen, pencil, ruler, calculator (if permitted) and I.D. card will be allowed on the student’s table.
  • No food, drinks, electronic devices - cellular telephones, laptop computers, programmable calculators, palm pilots, or pagers are permitted in the examination room.
  • No hats and sunglasses may be worn during the examination.
  • An invigilator will accompany students who need to use the washroom.
  • No communication, verbal or non-verbal, is permitted once the student enters the examination room and during the examination.
  • At the conclusion of an exam all writing within the answer books shall cease. Acceptance of the exams may be refused of candidates who fail to observe this requirement.
  • If an offence is committed, The Code of Behaviour on Academic Matters will be enforced including the loss of academic credit and expulsion.
