Personal Training
Personal Training and Strength ConsultationsAll personal training registrations must be done in person in the AC’s Main Office. Registrants are required to fill out a PARQ (Physical Activity Readiness Questionnaire) and a client profile form before registrations will be processed. Nutrition Packages
Please complete the nutrition profile form when you sign up in the Main Office for a nutrition consultation.
Renewal
When you renew, you do not need to complete your preliminary forms in person at the Main Office. You will need to complete a one-page renewal form indicating the additional number of sessions that you would like to purchase and indicate the professional’s name with whom you have been working.
Payment
Payment for sales and registrations may be made by cash, debit card, VISA or MasterCard. We gladly accept personalized cheques for amounts over $50 provided the cheques include the name and address of the account holder. A $35 fee will apply to any cheques returned by the bank.
Processing
Following registration, your completed forms will be directed to the Program Manager who supervises the service that you have requested. You will be matched with a qualified staff member who will contact you for your first appointment. You can expect to be called within three business days of completing your registration.
Cancellation
You must provide 24 business hours (9 a.m. - 5 p.m. Monday through Friday) notice to cancel a training session. Failure to do so will result in a loss of that training session.
Refunds
Refunds are processed only when a medical note is provided.
