Making An Appointment
To make an appointment, please contact the clinic receptionist at 978-4678, press “0”, or stay on the line for further information.All new patients are required to complete a Registration Form and Health History Form. In order to speed up the registration process you can download these forms and complete them prior to coming to your first visit.
Returning patients need to update any information on their record to ensure it is current.
All information gathered for treatment is confidential except as required or allowed by law or to facilitate diagnosis (assessment) or treatment. You will be asked to provide written authorization for release of any information.
New non-students need to obtain a referral from their family physician to book with one of the sport physicians. A direct referral for therapy from an external physician is acceptable.
Cancellation Policy
We require 24 hours notice (during business hours) for cancellation without penalty.Please note that the clinic does not treat patients with workplace or motor vehicle injuries.
Facility Supension
The Faculty reserves the right to deny entry to anyone who is in default of payment of clinic fees.| STUDENTS with non-exercise related health problems should consult: | ||

Fees
Currently registered U of T Students - An annual (May 1 – April 30) $60 administrative charge applies to all students for therapy services. Chiropractor visits are $10 per session.Non-U of T students - An $100 assessment fee applies to therapist visits and a $60 fee applies to all follow-up therapist visits. Chiropractor appointments are $100 per session.
You are required to pay for each session at the time of your visit.
The clinic receptionist will be able to provide you with the appropriate billing information. Please check your health care plan for your specific coverage.
Please note: A $35.00 fee will apply to any cheques returned by the bank.Health Insurance Coverage
Most physician medical services for Canadians are covered by provincial medical plans. International students receive coverage through the University Health Insurance Plan (UHIP) and other University of Toronto students are covered under university plans such as ETFS; the clinic accepts these plans directly depending on the period of coverage. Clients who have other private medical care plans will be required to pay for their medical services at the time of their appointment.Staff Directory
